Integrated Sexual Health Services remain available during lockdown. They continue to provide urgent and some non-urgent care to service users, however, there are a number of temporary changes in response to the coronavirus Covid-19 outbreak.

There currently isn’t the option to ‘walk-in’ and if you wish to access an appointment you will be triaged via their Information and Booking line: 0800 328 3383, Monday to Friday, 8am – 5pm.

You will be assessed over the phone and priority patients are given a time for either a face to face or a telephone consultation appointment.

Booked appointments can be made for the following clinics only: Chesterfield; Derby; Buxton; Swadlincote and Long Eaton. Other peripheral clinics are currently closed.

The service now has a number of self-care options that you can access online. These free, confidential and discreet items include:

• Condoms by post – C-Card for 13-24 year olds and over 24 years+.
• Oral contraception – virtual assessment and prescriptions posted out.
• Postal emergency contraception (over 16s only). Under 16s can still contact us to request emergency contraception via our information and booking line.
• STI & HIV testing – including treatment for chlamydia by post.
• Virtual sexual health promotion advice – via self-referral online to speak to a sexual health promotion practitioner regarding things like risky sexual behaviours, STIs concerns and general sexual health & wellbeing advice.
• Photo diagnosis for lumps, bumps and warts – for people aged 18 years and over. This can be booked via the information and booking line number.

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. 

This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

The CPCS Service was launched by NHS England on the 29^th of October 2019.

This was designed to facilitate patients having a same day appointment with their community pharmacist for a minor illness or an urgent supply of a regular medicine.

They have the aims of improving access to service and providing more convenient treatment close to the homes of patients.

You can access this service by referral through calling your GP practice, or by calling the NHS 111 line.

What can your Pharmacy do?

All pharmacists train for a minimum of 5 years, in medicine, minor illness and well-being, so therefore can offer clinical advice and over the counter medicines on a range of minor illnesses, such as coughs, colds, sore throat, bowel issues, aches and pains.

They do not require appointments, and have access to private consultation rooms, to help you discuss issues in private. If symptoms suggest it is more serious, pharmacists have the correct training to get you the help/ appointments you need, for example, GPs or nurse etc.

The On Day Service is running in the areas of Ilkeston and Long Eaton to improve access to primary care.

Patients who call their GP for an appointment may be directed on that same day to the On Day Service where they will be seen by  an advanced nurse practitioner or advanced care practitioner as appropriate.

The On Day Service is available from 8am to 6.30pm on weekdays, excluding bank holidays. To use it, patients just call their normal GP number.

What is an Advanced Nurse Practitioner / Advanced Care Practitioner?

Advanced Practitioners are highly trained, skilled clinicians who can help you with many different health problems.

“I decided to undertake extra training so I can help patients with more complex problems.

Now I can prescribe medicines, request tests and interpret the results, and work on treatment plans with patients.

I can help a lot more patients, freeing up the doctors to see patients with the most urgent or complicated health issues.”

Everyone between the ages of 40 and 74 will be offered a health check every five years and over 75’s will be offered one every 3 years. At the check the risk of heart disease, stroke, kidney disease and diabetes will be assessed and you will be offered support and advice to help reduce that risk.

All our doctors and nurses offer advice and support to patients trying to give up smoking.

Our nurses provide smoking cessation appointments where they provide advice, support, monitoring, nicotine replacement and advice on other techniques to help you quit.

Anyone who is aged 65 and over, persons in special categories eg diabetes, heart disease, asthma, COPD and who have had a splenectomy, are advised to have this vaccination. This is a one off injection which will give life long cover.

The flu vaccine is offered yearly to certain population groups, this is normally offered from September/October until February the following year.

You are eligible for a FREE flu vaccine if you:

• Are aged 50 or over
• Have a long term health condition such as a heart problem, diabetes etc.
• Have learning difficulties
• Are the main carer of an older or disabled person
• Are a frontline health or social care worker
• Are pregnant
• Are a child of a certain age

We will contact you to book your flu vaccine if you are eligible.

If you have diabetes you will have a review with a nurse to monitor your condition. The appointment may involve discussing your symptoms, a discussion about your medicines, breathing tests etc.

Your practice will contact you to book your diabetes review when it is due, but if you have any concerns about your condition in the meantime please contact the surgery.
